PREPARING FOR OUTDOOR SCIENCE SCHOOL

Medical Forms and Health Concerns

 

Medical Forms

Please make sure that all forms are filled out completely. The health forms require two signatures; plan an additional signature for the “Request for Medication” form. EVERYONE, including cabin leaders and teachers must have a health form.

Alphabetize the forms as a groupBEFORE arriving to the Outdoor Science School and clearly mark each health form in the upper right-hand corner with the following code if any such conditions exist.

Codes:

AL – Allergies requiring special meds.                           AS- Asthma                       

ADHD- Attention Deficit/Hyperactivity Disorder          BD- Birthday this week

BK- Back Trouble                                                          BS- Bee sting allergy

BT- Bowel Trouble                                                        BW- Bed wetter

CHF- Check health form                                                CT- See teacher for more info

D- ­ Diabetes                                                                  EP- Epilepsy

ESL- English is second language                                   FAL- Food allergy

HA- Headaches                                                              H of H­ – Hard of hearing

HRT- Heart Problems                                                    NB- Nose Bleeds

NESp- Non-English Speaking                                       NHF – No Health Form           

NP or No Pic­ – No photos allowed                              P- Parent is here

RX- Prescription meds                                                    SPN - Special needs in Education

ST- Sinus trouble                                                            SW- Sleep walker

V- Vomit Prone                                                               +G- These students should be together

!- History of behavior problems at school                        -G- These students should be separated

General Health

During your week at Outdoor Science School your student’s health is our number one concern. Try to do lice checks a week before you leave so that your students can be treated over the weekend and not miss out on Outdoor Science School. Also any student who has been sick with the flu or vomiting with in 24 hours of leaving for Outdoor Science School needs to stay home and get better. Making your students understand basic health precautions: washing hands, covering your cough by using the Dracula method (using your elbow), having a water bottle and the importance of keeping hydrated, and having chap- stick and using it to protect it from wind and sun burn will help their experience at Outdoor School be more enjoyable.
